1. DUTIES AND RESPONSIBILITIES

1. The    incumbent   checks    and    sequences   the    most    accurate   ICD-9/ ICD10- CM/CPT/HCPCS/DRG/Other codes for diagnoses and procedures for documented information.   Assures the final diagnoses and operative procedures as stated by the physician are valid and complete.
   1. Abstracts all necessary information from health records to identify secondary complications and co-morbid conditions.

2. Evaluates the record for documentation consistency and adequacy.  Ensures that the final diagnosis accurately reflects the care and treatment rendered. Computes and gives the correct DRG coding all inpatients cases.

3. Providing training and guiding other coders / Medical Records Technicians in coding, updating them with new coding rules and regulations as and when it is necessary.
   1. Analyzes   doctors’  documentation   to   assure  the   appropriate   Evaluation  & Management (E & M) levels are assigned using the correct CPT code.
   2. Ensures coding is as per HAAD guidelines and regulations.

4. Provides feedback to Doctors regarding coding errors or oversights.

5. Constantly updates to the latest coding versions and HAAD coding directives.

6. Performs miscellaneous job-related duties as assigned.

7. Performs any other jobs or duties assigned by the HOD from time to time within the scope of job title.

Responsibilities

• Checks and sequences the most accurate ICD-9/ICD-10-CM, CPT, HCPCS, DRG, and other codes for diagnoses and procedures based on documented information; ensures final diagnoses and operative procedures stated by the physician are valid and complete.
• Abstracts necessary information from health records to identify secondary complications and co-morbid conditions.
• Evaluates records for documentation consistency and adequacy; ensures final diagnosis accurately reflects care and treatment rendered.
• Computes and assigns correct DRG coding for all inpatient cases.
• Provides training and guidance to other coders/Medical Records Technicians, updating them on new coding rules and regulations as needed.
• Analyzes doctors’ documentation to ensure appropriate Evaluation & Management (E&M) levels are assigned using correct CPT codes.
• Ensures coding compliance with HAAD guidelines and regulations.

• Keeps updated with the latest coding versions and HAAD coding directives.
• Performs miscellaneous job-related duties as assigned.
• Executes any other duties assigned by the HOD within the scope of the job title.

Qualifications

• Bachelor’s degree in Allied Health Sciences or related field.
• Minimum of 2 years of coding experience.
• Valid Certified Coding Associate (CCA) certification from AHIMA or Certified Professional Coder (CPC) certification from AAPC.
• Proficient in computer applications, including MS Office.
• Excellent command of spoken and written English.
