
Job Description
KEY RESPONSIBILITIES
- Reviews and interprets medications orders (verbal and written) for clarity, rationality, dosage form, allergies, interactions, incompatibility, etc.
- Communicate with the healthcare provider for clarification and discrepancies about the medication order and document the intervention properly.
- Transcribes to computerized patient medication profiles accurately, and maintain complete patient medication profiles.
- Report medication errors.
- Provides drug information and assists in instructing nursing and medical staff and pharmacy students.
- Performs inspection of nursing units, discusses service problems, and recommends solutions to Pharmacy Supervisor.
- Develops and co-ordinates the Quality Improvement Program within his/her area of responsibilities.
- Fulfill the requirement of 24 hours pharmacy service within the Pharmacy schedule.
- Attend the pharmacy continuing education program and departmental meetings
- Serves as a role model and represents the department in a professional manner.
- Cooperative, initiative, motivated and consider others perspectives.
- Participate in patient and family education to help patients and families to better understand and participate in their care and make well informed care decisions.
- Performs other applicable tasks assigned within the level of his competence as required by the title of the post.
Necessary Knowledge and Experience
Essential: Experience and knowledge of regulations and standards of pharmacy practice is required.
Desirable: More than two (2) years experience in Hospital pharmacy.
Education and Certification Requirements
Bachelor’s Degree in Pharmacy from an accredited college of pharmacy required with a valid professional registration certificate.
Skills
Essential: Knowledge of computerized pharmacy system, I.V admixture and unit dose system.
Desirable: Knowledge of communication skills and ability to counsel patients. Previous working experience in Middle East healthcare facilities.
